HEALTH FORMS

forms REQUIRED BY THE STATE OF MARYLAND

The state of Maryland regulates summer camp. In compliance with state law, all campers must have completed health forms prior to participation.

If a camper does not attend school in Maryland, even they are a Maryland resident, vaccination records are required and must be submitted before your child can attend camp. If we do not receive the forms until the morning of your camper's first day, please note that, by law, your child must stay in the gym lobby until his or her forms are processed at the front desk. 

If your child requires special medication needs, additional forms are necessary (see Additional Health Forms, below).

CAMPERS WILL NOT BE ADMITTED WITHOUT THE REQUIRED FORMS AS THIS VIOLATES MARYLAND STATE LAW.

ADDITIONAL health forms

The following forms are also required, in the following cases:

  1. If your child resides outside of Maryland, or is a Maryland resident but attends school outside of Maryland, please submit:

    • Your child's Health Immunization Record

  2. If your child will self-administer medication or requires emergency medication during camp, please submit:

    • Medication Administration Authorization

    • Emergency Care for Management of Anaphylaxis

  3. If your child has special health needs, please have your physician complete:

    • Section 504

Registration PROCESS

We know signing up for summer camps can be a hassle, so we've made it easier!  Here are the steps:

  1. Click on the PARENT PORTAL link below and create an account on our parent portal/add student(s)

  2. Click on “Booking”

  3. Click on “Limited Summer Camp”

  4. Select the camp type and camp weeks you are requesting. PLEASE NOTE: This year camp enrollments are not automatically approved, if you are signing up within a few days of your camp start date it may be declined. It may also be declined due to our limited space. Please only come to camp if your camp enrollment has been approved.

  5. Add payment info in parent portal and pay.

  6. Click on HEALTH FORMS link below (these forms are REQUIRED for participation)

  7. See ADDITIONAL FORMS section below if your camper attends school outside of Maryland and/or has allergies or other medical requirements.

  8. All forms MUST be processed before campers are permitted in the gym. Registration and Health Forms can be completed online; Additional Forms can be emailed to hello@theagilitycenter.com.
